Transcription Services
Description: ApexScribe's transcription services include receiving, typing or editing, formatting, and distributing electronic copies of health provider dictated medical documents. We convert the providers' voice recordings into electronic medical record documents.
Providers can select from a variety of options for capturing their dictation, including telephones, PDAs, and desktop computer-based dictation devices. Typically the health provider will dictate patient findings into a digital voice recorder or 1-800 telephone number. We capture these voice files and convert them to text documents which are typically within 18 hours either via our secure web based portal or directly into the provider's server.
Physicians can edit, approve, search, and archive these records via a browser. Our services are HIPAA compliant.
Benefits: Provider productivity is increased. Apexscribe charges competitive rates for such services and is able to offer shortened turn around time for document delivery. Apexscribe's transcription solution lowers operating costs by 20-30%. We offer web-based centralized administration and automatic document routing.
Document Management Services
Description: Because ApexScribe is Web-based, it offers easy-to-use tools to manage documents, users and workflow from any computer with Internet access, creating numerous opportunities for productivity improvement.
Benefits: Costs related to lost paper records and delayed reports (which delays medical billing and reimbursement) are eliminated. Documents are stored digitally and backups can be made periodically.
Performance Assessment - Medical Records Documentation
Description: Medical documentation of patient complaint(s) and treatment must be consistent, concise and comprehensive. Many healthcare providers use the SOAP note format to standardize medical evaluation entries made in clinical records. The letters S-O-A-P stand for SUBJECTIVE, OBJECTIVE, ASSESSMENT and PLAN.
The SOAP note is written to improve communication among all those caring for the patient. By using a standard format amongst providers you can display the assessment, problems and plans in an organized format to facilitate the care of the patient.

Clinical Intelligence - Data Extraction
Description: Apexscribe can accurately extract relevant information such as medication, allergies, and problems from the transcribed documents that flow through its transcription service. This relevant information can include such things as drug names to recognize the context in which the drugs are mentioned: the patient currently takes, previously took, stopped taking, is allergic to, etc.
Benefits: The potential benefits that this data represents are tremendous. Currently, healthcare providers are spending anywhere from $0.80 to $2.5 per patient document to extract such information. Apexscribe can do the same extraction at a fraction of the cost in a fraction of the time.
